Primary Health Care

Patients at Ill and Well Child Clinic should be registered to see a selected (in the declaration) doctor. The selection principle is not applicable only in exceptional cases (doctor's absence, holiday or sickness).

Services provided within Primary Health Care:

  • Well Child Clinic

  • Ill Child Clinic

  • Vaccination Centre. We offer a full range of vaccinations (obligatory and recommended). We have all the vaccinations now available on the market.

Primary Health Care patients may continue their treatment in our outpatient specialist clinics and make use of the hospital laboratory. For the issuance of a continuation referral to a specialist clinic and the printouts of test results, please write to: poz@dzieciecyszpital.pl (WE DO NOT MAKE APPOINTMENTS VIA E-MAIL. APPOINTMENTS ARE MADE ONLY BY TELEPHONE). Please state the child's name, surname, pesel number or date of birth as well as the doctor's name. The collection of the abovementioned up to 3 business days at the registration. Test results will be duly annotated having been checked by the practitioner.

First-time patients must fill in a declaration downloadable below or at the General Registration.
